Our phone lines remain open for anyone with accessibility needs whom might find difficulty completing this form. For those without accessibility needs, our receptionist may direct callers to complete this online form.

Common queries

If you have a medication query, please submit an admin request online. Click 'Sick notes, test results or administrative help'. Please provide as much detail as possible.

If you have a test result query, please submit an admin request online. You can still call the practice to find out your results, however, we may not have received the results yet.

If you need to change or cancel your appointment, please submit an admin request online. If you are unable to submit an online request, please call our reception team.
